Schedule: Monday–Friday, 8:00 AM–5:00 PM
Compensation: $65,000–$80,000 base salary + uncapped commission
About the Role
We are seeking a compassionate, self-driven, and community-focused professional to join our team as a Community Liaison. In this role, you will serve as the face of our organization in the San Antonio community, promoting our specialized end-of-life care services and helping individuals and families access the compassionate support they deserve.
The Community Liaison is responsible for building and maintaining meaningful, trust-based relationships with physicians, hospital discharge planners, skilled nursing facilities, community agencies, local organizations, caregivers, and other referral partners. You will educate prospective referral sources, patients, families, and community members about our philosophy of care while supporting referral growth, census development, and strong community awareness.

Key Responsibilities
- Establish, develop, and maintain strong professional relationships with physicians, hospital discharge planners, skilled nursing facilities, healthcare professionals, community agencies, local organizations, and other referral sources.
- Conduct regular, intentional outreach and relationship-building visits throughout the assigned service area.
- Promote awareness of our specialized end-of-life care services through meetings, educational presentations, community events, networking opportunities, and one-on-one outreach.
- Serve as an advocate for our organization by clearly communicating our philosophy, services, and approach to prospective referral sources, patients, families, caregivers, and community members.
- Lead initial conversations with patients and families regarding available care services, helping them make informed decisions with empathy, professionalism, and respect.
- Respond promptly to inquiries from prospective clients, caregivers, referral partners, and community members, connecting individuals to appropriate services and resources.
- Drive referral growth and census expansion by positioning the organization as a trusted provider of choice within the community.
- Attend local community events, healthcare events, networking functions, and partner meetings to strengthen visibility and identify new relationship opportunities.
- Collaborate closely with internal clinical, operations, admissions, and leadership teams to ensure consistent messaging, coordinated follow-up, and an exceptional experience for patients, families, and referral partners.
- Track outreach activities, referral-source interactions, community events, and business-development efforts in accordance with company expectations.
- Provide feedback on community needs, referral trends, market opportunities, and engagement strategies to support continuous improvement.
- Maintain strong, respectful working relationships with key staff, community agencies, healthcare providers, and organizational partners.
Required Qualifications
- Bachelor’s degree in a related field, or equivalent professional experience.
- Minimum of 1 years of sales or marketing experience within the healthcare industry.
- Proven ability to establish, develop, and maintain professional relationships with diverse stakeholders.
- Strong community engagement and community outreach skills.
- Excellent communication, interpersonal, presentation, and customer-service abilities.
- Ability to communicate sensitively and effectively with patients, families, caregivers, healthcare providers, and community partners.
- Strong organizational, time-management, and follow-up skills, with the ability to coordinate meetings, events, outreach activities, and relationship-development efforts.
- Valid driver’s license, reliable transportation, and current auto insurance.
- Ability and willingness to travel locally throughout the San Antonio service area for community events, partner visits, and referral-source meetings.
Preferred Qualifications
- Experience in hospice, palliative care, home care, home health, or other healthcare-related marketing.
- Familiarity with the needs of patients, families, caregivers, and individuals navigating healthcare or supportive-care services.
- Prior clinical, social services, community relations, public relations, or healthcare background.
- Experience serving as a community liaison, account representative, outreach coordinator, admissions liaison, or business-development professional.
- Track record of exceeding sales, referral-growth, census-growth, or relationship-development targets.
- Post-secondary education in social work, communications, public relations, healthcare administration, or a related field.
